Instructions For Viewing, Downloading, & Submitting Forms

Microsoft Word is required in order to display, fill out and print the forms

Viewing the Form

• To view the form, click on its hyperlinked name.
• Depending on what computer and operating system you use, you might see the form in your Web browser's window, or the form might appear in a separate window. In either case, you can then use Microsoft Word to fill out the fields in the form and print the form on your PC's printer.

Downloading and Saving a copy of the form on your Computer

• With this Web page in your browser window, right-click on the form's hyperlinked name.
• From the context menu that appears, select "Save Target As....".
• In the Save As dialog box, choose a directory to save the file in, then click "Save."

Submitting the Form

• Because forms require a handwritten signature, they must be printed and manually submitted.
